- CSC Center (Jan Seva Kendra)- To make Ayushman card you need to visit the nearest CSC center (Jan Seva Kendra). Please bring your Aadhar Card and Mobile number. Tell the CSC Center owner to make Ayushman Card. The CSC operator will register you on official portal https://beneficiary.nha.gov.in/. Once done you can download and get the Ayushman Bharat card.
- Go to the listed hospital and use the Ayushman Mitra Service- You can Apply for the Ayushman Card directly from listed hospitals (In case of emergency) , you can get more information in hospitals Ayushman card counter. Please carry your Aadhar card and mobile with you while visiting the hospital (listed).
- Register yourself using the Ayushman Bharat Card Official mobile Application.

- First, you have to visit the official Portal at https://beneficiary.nha.gov.in/
- On the home page, you will see the login box.
- Now, enter your “Mobile Number” and click on “verify” option.
- Once done, you will get OTP on your mobile.
- After that, verify the OTP.
- Now, select Auth mode as “mobile OTP” from dropdown.
- Once done, verify the captcha code.
- Once done, click on login button.

- First, you have to visit the official Portal at https://beneficiary.nha.gov.in/
- On the right home page, you will see the login box.
- Now, enter your “Mobile Number” and click on “verify” option.
- Once done, you will get OTP on your mobile.
